​Plympton Vet Clinic

We are looking forward to having your pet board with us at Plympton Vet Clinic


Thank you for choosing to board your pet at Plympton Vet Clinic.


Plympton Vet Clinic Boarding Form

This is the Boarding Admission Form that is needed for every pet that is booked for boarding at our clinic.  This form is filled in after you have made the initial booking for our Clinic and paid the 2 day deposit.

EVERY pet needs a separate Boarding Admission Form completed.

Please make sure you upload all attachments if requested to do so (note - if your pet is a client with us at Southern Animal Hospital, Marion Animal Hospital or here, at Plympton Vet clinic there is no need to upload attachments as we can access your pet's information).

  • Please submit this form as soon as possible to complete your boarding booking.  We do kindly request it be returned at least 7 days prior to admission date.

  • On the day of admission, you will be asked to check all details and sign our boarding agreement.  You should allow 10-15 minutes for drop off and pick up of your pet(s) so we can go through your form, check your pet's belongings are all labelled and clarify anything else you need to know.  

  • No pets will be accepted into boarding without this form being completed and relevant documentation being provided (if requested to do so).


IMPORTANT NOTE - Plympton Vet Clinic is closed on Sundays and all Public Holidays.  Staff cannot be contacted via phone.   If you have arranged for your pet to be dropped off or picked up on a holiday or a Sunday, please make sure you are aware of the limited time you can do so and know where to go in advance (Lincoln Avenue side gate).   If not sure, we ask that you speak with staff in advance of your boarding dates.

Older Pet Clearance Form 

This form is to be filled in by your vet if requested to by Plympton Vet Clinic. This is for any pet who is 8 years and over and not a Southern Animal Hospital Group client. 

Please download the file below and send to your vet and ask them to complete.  You can then upload the completed form as part of the Boarding Admission Form.  If your vet is not able to fill in this form, you will need to contact our Clinic and make an appointment for a consultation with a vet.

​Why is an older pet clearance form needed?    

For the safety and wellbeing of senior pets, we require an Older Pet Clearance Form for any pet over the age of 8 who is not part of our veterinary group (ie. not a client from Southern Animal Hospital, Marion Animal Hospital or Plympton Vet Clinic)

Boarding can be more physically and emotionally demanding for older animals, as they may struggle with changes to routine, unfamiliar environments, or increased activity around other pets. Senior pets are also more prone to health issues, and the stress of boarding can sometimes worsen existing conditions. This form, completed by your veterinarian, confirms your pet is fit to board and provides us with essential medical information. This is a common requirement across many boarding facilities, and helps ensure we can provide safe, informed care during your pet’s stay.

​Pets under our veterinary care do not require this form, as we can access their records directly.


What if my own vet will not give me this clearance form?


If your vet is unable to fill in this form or provide any other written clearance, you can make an appointment with one of our vet team prior to the boarding dates. Consultation fees will apply.
